AIEP Enrollment Forms
Parents and guardians who would like to have their student(s) join the AIEP please complete the AIEP Student Registration Form to join the program. Students cannot attend classes, field trips and events, or receive services until this form is completed. Please fill out the form for each new student.
District Staff are encouraged to share this form with eligible families.
AIEP Student Registration Form
Students who are enrolled in a federally recognized tribe or have a parent/grandparent who is enrolled in a federally recognized tribe are encouraged to fill out the 506 form. This form helps maintain our funding but is not required to join the program.
