    ONLINE REGISTRATION ⇒

    Fall Sports Registration NOW OPEN!

Please read and follow these steps carefully to be fully registered!

IMPORTANT:  You are only registered once ALL steps are completed

Step 1: All students (grades 7-12) who plan to participate in ATHLETICS must have a current physical examination on file (within the last 3 years) before the start of each season.*  Per state law & the MSHSL, physical examinations that expire result in immediate ineligibility until a new physical exam has taken place. Physical exams must be performed by a duly registered physican, physician's assistant, or nurse practitioner. Athletes WILL NOT be allowed to participate without having a physical exam in good standing. You can check your phsycial date through your Portal account under the health tab.  If needed, you can get the current blank physical form HERE.

Step 2:  View the Activities Eligibility Brochure carefully.

Step 3: Fill out and submit the Online Registration Form here (includes all forms in one).

Step 4:  Pay your participation fee. Contact the Activities Office if you are unable to pay online.

    *Extension of Expiring Physicals:

    • The MSHSL Board of Directors Executive Committee approved the extension of the expiration date of the Three-Year Physical Clearance.
    • A student who has participated in an MSHSL sport in ISD728, and has a three-year clearance (physical) on file that expires at any time from March 12th, 2020 through the conclusion of the 2020-2021 school year will have the clearance extended through the completion of the 2020-2021 school year.
    • Any Student who has not previously participated in an ISD728 MSHSL sport or does not have a physical on file with medical clearance to participate will be required to submit a new (or current) physical to the Zimmerman High School Activities Office.  This is required in order to be cleared to participate.

    IMPORTANT TRANSPORTATION INFORMATION!

Due to COVID-19 and social distancing limitations, there will likely be a shortage of school transportation during the 2020-2021 school year.  In the event of a shortage, ISD 728 may not have the ability to provide transportation for athletic and activity programs.  As a result, it will be the responsibility of the parent, guardian, or student to arrange their own transportation.  This may include transportation to practices and/or contests.  ISD 728 policy states that coaches may not transport students in their personal vehicles or arrange private transportation for students. The District will make every effort to provide transportation, however, outside factors may prevent us from doing so this school year.

When registering you will be asked to acknowledge that transportation may not be provided and waive ISD 728 of liability.
