Supporting Student Health & Learning in ISD 728
The goal of the ISD 728 Health Services Department is to create and sustain a safe and healthy school environment that supports student achievement and well-being. Research shows that healthy students are better learners—and we’re here to help every student thrive.
Our Health Services team works closely with students, families, and school staff to promote student wellness and ensure access to care during the school day.
What Do Health Services Staff Do?

Health Services staff are essential members of each school’s support team. Together, we:

Promote wellness and healthy habits
-
Monitor and update immunization records
-
Develop and manage health care plans for students with medical conditions

Provide first aid, illness care, and emergency response
-
Administer medication as prescribed
-
Implement state-mandated health screenings
-
Track communicable diseases
-
Maintain health records and student care documentation
-
Encourage self-care and independence in students
Who Makes Up the Health Services Team?
Licensed School Nurse (LSN)
An LSN is a Registered Nurse (RN) with a bachelor’s degree and licenses in public health and school nursing. The LSN supports students with medical and mental health needs through assessment, care planning, coordination, and training.
Key responsibilities:

Develops individualized health plans (IHPs)

Assesses student health needs and provides consultation

Delegates and supervises medication administration
-
Trains and supports staff working with students with health needs
-
Participates in 504 Plan and IEP teams
-
Connects families to community health resources
Health Services Assistant (HSA)
HSAs are trained in CPR/AED and First Aid and work under the supervision of the LSN. They support daily student health needs at school.
Key responsibilities:

Provide first aid and care for illness or injury

Administer medications as delegated by the LSN

Maintain student health records and track immunizations
-
Support health screenings and health office procedures
Communication is Key
If your child has a chronic condition or requires medication, treatments, or accommodations at school, please contact your school's Health Office. This helps us plan ahead and provide the best care.
Complete the appropriate health forms and return them to your school’s Health Office as soon as possible to support a safe and successful school experience.
