Student Health Conditions & Action Plans
To support student health, safety, and academic success, our school district partners with families and healthcare providers to ensure that students with chronic health conditions have the care they need throughout the school day.
If your child has a diagnosed health condition, such as asthma, diabetes, seizures, or life-threatening allergies (anaphylaxis), an individualized Action Plan is required for school.
These action plans guide school personnel in recognizing symptoms, administering treatment promptly, and helping your child return to learning as quickly and safely as possible. Having a current, physician-approved plan helps minimize classroom disruption, avoid unnecessary emergency room visits, and ensures your child’s needs are met in a consistent and coordinated way.
What Families Need to Know

Each Action Plan must be:

Completed and signed by both a licensed health care provider and a parent/guardian

Updated annually, or sooner if your child’s condition or treatment changes

The plan should include:

Clear steps to take when symptoms occur

Emergency contacts

Any required medications and instructions for use

Please submit your child’s completed Action Plan to the Health Services Office at their school as soon as possible—even if medication will not be stored or administered at school

If medication is needed at school, it must be:

Provided in the original container or prescription-labeled packaging

Accompanied by a completed medication authorization form

If your child's health condition is no longer a concern or has resolved, a parent or guardian must contact the Health Services Office to request removal of the condition from the student’s health record. This helps us keep your child’s medical information current and accurate.
