Immunization Compliance
To protect students from preventable communicable diseases, Minnesota State Law (M.S. 121A.15) and ISD 728 Student Immunization Policy 530 require that all students enrolled in school provide documentation of required immunizations or appropriate exemption paperwork.
What Families Need to Know
Requirements for Kindergarten - 12th Grade Children

Proof of Immunization is Required: Prior to the first day of school, each student must submit one of the following:

Documentation showing completion or in-progress status of all required immunizations

A legal exemption (medical or conscientious belief), signed and notarized per state law
-
If your family is submitting a medical or conscientious objection to immunizations and does not have access to a notary, please contact the Health Services Senior Administrative Assistant for guidance and support.

Students who are not in compliance by the first day of school will be excluded from attending school until proper documentation is provided.

Submitted via alternative immunization records (such as from a clinic or electronic health record), as long as all state-required vaccines are clearly listed

Requirements for Early Childhood and Preschool-Aged Children
Minnesota immunization law also applies to early childhood education programs, including:
Programs that provide instructional or other services to support children from birth to kindergarten who meet at least once per week for six weeks or more during the school year.
Students enrolled in these programs must also provide:

A legal exemption, documented on the same immunization form
These requirements apply to all early learning programs operated by or in partnership with the school district.
Required Immunizations
The list of required vaccines may vary slightly by grade level but typically includes:

DTaP (Diphtheria, Tetanus, Pertussis)

Polio
-
MMR (Measles, Mumps, Rubella)
-
Hepatitis B
-
Varicella (Chickenpox)
-
Meningococcal (including booster for 12th grade)
-
Hib, Hepatitis A, and Pneumococcal (for early childhood programs)

Exemptions
Minnesota law allows exemptions to required immunizations under specific conditions. No student is required to receive a vaccine if:

There is a medical contraindication, or
-
Laboratory evidence of immunity exists, or
-
The immunization is contrary to the conscientiously held beliefs of the parent or guardian
To qualify for an exemption:

Medical Exemptions must:

Be signed by a licensed health care provider

Clearly state the specific vaccine(s) being exempted and the medical reason

Conscientious Exemptions must:

Be signed by the parent or guardian
-
Be notarized
-
Identify the specific vaccine(s) being declined due to personal beliefs

Exemptions must be submitted on the official Minnesota Immunization Form, available at your child’s school or are available here:
All exemptions are reviewed and processed by the school’s Health Services personnel to ensure compliance with state law.

Physical Exams & Sports Physicals
Regular physical exams play an important role in a student’s overall well-being and success in school. These exams help identify medical, developmental, and emotional concerns early, allowing families and school staff to work together to ensure students are healthy, safe, and supported.
General Physical Exams
It is recommended that students receive a complete physical examination before entering the following grade levels:

Kindergarten

4th Grade

7th Grade
-
10th Grade
These routine wellness checks allow healthcare providers to monitor your child’s growth, development, and health status and to update immunizations as needed.
Physical exam forms completed by your healthcare provider can be submitted to the school health office.
Sports Physicals for High School Athletes
In order to participate in high school sports, students must have a current sports physical on file, as required by the Minnesota State High School League (MSHSL).
Key Requirements:

Sports physicals are valid for three years, but must remain current for the entire season of participation

It is the family’s responsibility to ensure that the physical is current before tryouts or the first day of practice

Completed sports physical forms must be submitted to the school’s Activities Office, not the Health Office

When Health Changes Occur
It is highly encouraged that families inform the Health Services Office if a student has a change in physical health resulting from a well-child visit, injury, or new diagnosis. This helps school health personnel:

Prepare for any needed accommodations

Monitor symptoms or medications during the school day
-
Provide safe support during classes, activities, or transitions
Even if the change does not require medication at school, it’s still helpful for staff to be aware so they can assist your child if needed.
