Vision & Hearing Screenings
At ISD 728, we are committed to supporting the health and academic success of every student. Vision and hearing are critical to learning, communication, and classroom engagement.
Screening Program Update
ISD 728 is no longer scheduling mass vision and hearing screenings during the school year.
However, if you have a concern regarding your child’s vision and/or hearing, a screening can be scheduled upon request.
How to Request a Screening
To request a vision or hearing screening for your child:

Complete the Online Request Form
OR -
Contact your child’s school Health Office directly to make a request.
Screenings will be performed by request only or as permitted during individual health evaluations with documented parental permission.
What Happens After a Screening?
If your child does not pass a vision or hearing screening, you will receive a referral notice from the licensed school nurse. The referral will include:

The area of concern (vision or hearing)

Specific screening results and findings

A recommendation to follow up with your child’s licensed healthcare provider for further evaluation or treatment
We encourage families to follow through with referrals to ensure students receive the care they need to succeed both in and out of the classroom.
