Sherburne County Vaccination Opportunity at Elk River High School

ISD 728 Families,

Sherburne County Health and Human Services is hosting a COVID-19 Vaccination event on Friday, April 30, 2021 at Elk River High School from 1:00 p.m. to 6:00 p.m. This vaccination opportunity is available to all students in ISD 728 ages 16 and older, as well as to interested parents/guardians of students. Minors (students who are sixteen or seventeen years old) attending a vaccination appointment will require a parent/guardian present to authorize permission for them to receive the vaccination. Parents/guardians attending with their child can schedule a time for a vaccination appointment as well. 

Minnesota County Health programs are setting up similar opportunities in schools across the State. This event, put on by Sherburne County Health and Human Services, is open to ALL ISD 728 families even if they do not live in Sherburne County.


If you would like to schedule an appointment, please read the following from Sherburne County Health and Human Services:

DO NOT SCHEDULE AN APPOINTMENT IF:

  • You are 15 years old or younger.
  • You have been exposed to COVID-19 and will be in quarantine during the clinic date.
  • You are positive for COVID-19 and in isolation during the clinic date.
  • You have had any vaccine administered in the 14 days prior to the clinic date.
  • You are unable to attend the Dose Two clinic date 21 days after the 1st dose, Friday, May 21, 2021, also at Elk River High School. 

 Schedule Your Appointment

  • Scroll down to see all available appointments. If all appointments are full, you may add yourself to the waitlist and you will be contacted.
  • When you get to the ‘Health Insurance’ screen, choose “No Insurance” in the ‘Insurance Type’ dropdown. 
  • Make sure you do NOT stop at the Review Screen!  Click Save and Continue at the bottom of each screen until you get to the confirmation screen that can be printed for your records.
  • Once your appointment is scheduled, you will receive a confirmation email from “Vaccination Clinics”.  If you do not see this in your Inbox, be sure to check your Junk mail. **the County system does not always send confirmation emails for some reason, but if you get to the scheduling screen that says you are getting a confirmation email, you are scheduled. If you want to call the COVID hotline number to confirm, please feel free to do so.**
  • If needed, you can cancel your appointment using a link at the bottom of your confirmation email.
  • Each person wanting a vaccination must have an appointment. 
  • If you are 16 or 17 years old, your parent or guardian must be present for you to get vaccinated.

Arrival at the Elk River High School

  • Address: 900 School St NW, Elk River, MN 55330
  • Park in the main parking lot off of School St.
  • Enter the Main Door (Door A) and look for the Registration table

 
The Day of Your Appointment

  • You should be feeling well.  If not, cancel your appointment and contact covid@co.sherburne.mn.us or 763-765-4156 to reschedule.
  • Before coming to your appointment, be sure to have something to eat.
  • Wear a shirt that allows easy accessibility to your upper arm.  A t-shirt is great!
  • You will be asked a series of health screening questions prior to entry.
  • The vaccination itself will only take a few moments but you will be required to stay on-site for 15-30 minutes afterwards for observation.
  • Please bring some form of identification.
  • If you are 16 or 17 years old, your parent or guardian must be present for you to get vaccinated.

 
Follow-Up

  • Sign up for V-Safe – a smartphone-based tool that provides personalized health check-ins after you receive a COVID-19 vaccination. There will be registration instructions available to you at the clinic.
  • Save the date for your second dose 21 days after your first dose (May 21, 2021). A scheduling email will be sent out to your email after your 1st dose appointment.
  • Fourteen days after receiving your second dose, you will not need to quarantine if determined to be in close contact with a person who has tested positive for COVID-19 per Minnesota Department of Health Guidelines.
